About the Role

We are seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Home Health Care Coordinator (RN) Remote. Job Summary

The Home Health Care Coordinator (RN) Remote will be responsible for supporting our Washington State Plan, working closely with multidisciplinary teams to assess, facilitate, plan, and coordinate care for members with high need potential. This role requires a strong understanding of behavioral health services, substance abuse, physical health/disease management, and long-term care. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with members, caregivers, physicians, and other healthcare professionals.
