The Opportunity
ColigoMed is hiring experienced Healthcare Assistants to join our Remote Patient Monitoring team!

ColigoMed is hiring Healthcare Assistants to join our US based Remote Patient Monitoring (RPM) team for Chronic Care Management. The role is focused primarily on new patient enrollment and helping patients to understand the value of the programs being offered and arrange for their enrolment consent.
The healthcare assistant (HA) works with care coordinator nursing staff to perform enrolment services and the role also supports the remote care management for chronically ill patients and collaborating with patients, their family and caregiver(s), other healthcare providers and community resources to implement approved health management plans. The healthcare assistant reports to care coordinators and is responsible for providing remote support to lead enrollment of new patients and also monitor a designated list of patients with chronic conditions to check on adherence to treatment plans.
The healthcare assistant remotely checks on patient progress against set goals and guidelines in order to help them receive education, tips, and better disease management information. If issues are identified then these are escalated to a care coordinator. The role does not give medical advice, diagnose, clinically assess or evaluate the patient which is done by the licensed providers.
This role also requires both fluent English and high-level interpersonal skills for dealing effectively with patients, providers as well as colleagues across various locations.
Candidates must be willing to work hours that provide an overlap period with the US Time Zone (EST) from 8.00am to 5.00pm Mon to Friday. This is mandatory to meet our job requirements.
Responsibilities
Build effective relationships and rapport with patients and is an advocate for patient health.
Receive referrals from contracted practices for new patients to be enrolled in remote care services and initiates phone contact with eligible Medicare patients
Provide patients with a clear understanding of the RPM program covering process, expectations, requirements and obtain consent for enrolment in programs
Activate newly enrolled patients in the ColigoMed portal.
Assist patients with downloading the ColigoMed application.
Order devices for remote patient monitoring (RPM) services.
Consult with RPM patients once they receive devices to assist them with their use and obtain initial readings.
Schedule an introductory call between patients and care coordinators.
Complete a schedule of patient monitoring calls each month to monitor patient adherence to treatment plans and patient progress per Center for Medicare and Medicaid (CMS) guidelines.
Keep the care coordinators informed of all issues pertinent to the treatment plan as needed and provide assistance with patient needs
Coordinate continuity of patient care with other healthcare facilities and care providers
Document all communications (verbal or written) accurately on the Coligomed platform
Adhere to documentation protocols and best practices for daily work logs, coding information for billing, escalation of patient issues and internal communications as needed using the Coligomed platform
Contribute as a positive and supportive member of the Coligomed team
Develop and maintain a strong knowledge of the remote care model and Coligomed platform and new developments in remote RPM and CCM solutions

Job Requirements:
Education/Experience
Medical Assistant with active certification is preferred, however, not mandatory
2+ years of relevant healthcare experience
Knowledge and Skills
Comprehension and communication skills with commitment to provide uninterrupted services and the ability to work remotely & effectively to receive and convey information
Organizational skills including prioritizing, scheduling, time management, and meeting deadlines
Ability to build relationships and earn trust with patients and healthcare providers and to manage changing priorities in an effective and organized manner
Maintain patients privacy, confidentiality and adhere to ethical and legal requirements
Experience working with a computer digital platform for RPM and CCM is preferred
High level of fluency in spoken & written English is mandatory with proficiency in other languages a plus
Note:
A background check will be required and after successful training, this position is a remote (from home) role.
You must have a dedicated work area established at home that is separated from other living areas and provides information privacy to perform professional, quiet, and confidential work.
You must live in a location that can access a high-speed internet service
As requirements and patient location evolves over time, flexibility in shift working hours may be required.
In terms of commencing salary, the level will take into account an assessment based on factors including the candidate’s level of experience, education, skills, and the company’s budgetary constraints.
