Role Description
This is a fixed-term contract role covering maternity leave across the Health Coaching team from December 2026 to approximately May 2027. This is a role for a skilled health coach who can hit the ground running in a fast-paced telehealth environment. You'll join an established team and take on a patient caseload from day one, delivering high-quality personalized coaching that drives real patient outcomes.
Patient Coaching & Behaviour Change
• Own and manage a caseload of patients and deliver personalized 1:1 (and occasionally live group) coaching that drives sustainable behavioural change aligned with clinical plans, resulting in patient satisfaction and retention that meets expected KPIs.
• Proactively identify opportunities to engage patients and drive behavioural change based on their clinical needs, milestone moments, and coaching judgment.
• Support in designing and adapting coaching interventions for patients, ensuring high engagement and giving feedback on any concerns or opportunities for improvement.
• Test new interventions that drive retention and satisfaction outcomes, and share learnings with the team.
Patient Experience & Service Quality
• Respond to patient queries and direct concerns to appropriate internal teams within agreed SLAs and metrics.
• Action patient feedback to deliver a higher quality service to drive higher patient satisfaction.
• Maintain accurate, timely documentation to support continuity of care and quality assurance.
• Buddy with new starters as required to mentor on how to coach safely and to a high quality.
• Monitor your own performance data and patient outcomes, and use insights to proactively adjust coaching approaches and identify patients at risk of disengaging.
Clinical Safety & Governance
• Participate in conducting auditing and feedback from peers.
• Monitor patient safety risks and escalate clinical concerns promptly in collaboration with the clinical team as per escalation protocols.
Commercial & Cross-Functional Contribution
• Identify opportunities to introduce relevant services and products offered by Juniper that align with patient goals and treatment progression, ensuring recommendations are clinically appropriate and patient-centred.
• Collaborate and participate cross-functionally to offer coaching and dietitian support in the broader care journey (for example, participating in projects with the shop, community or PES).
• Proactively surface patient insights with your team and business to inform service improvements.
Qualifications
• Accredited Practicing Dietitian (essential), with a strong grounding in medicated weight management.
• Experience managing a patient caseload and delivering high-quality coaching from day one.
• Deep understanding of motivational interviewing and behaviour change frameworks.
