Job Summary and Responsibilities

As a Home Care Practical Nurse at Amedisys, you will play a vital role in providing high-quality care to patients in their homes. Your responsibilities will include:

• Verifying the plan of care with the RN prior to each visit
• Collaborating with physicians and RNs to ensure patient needs are being met and notifying them of any changes in patient condition
• Teaching patients and caregivers self-care techniques
• Checking vital signs
• Reviewing all medications and performing medication reconciliation at each visit
• Performing specialized procedures ordered by physicians (e.g., foley catheter change)
• Preparing equipment and materials for treatments using aseptic technique
• Completing documentation timely and accurately

Requirements and Qualifications

To be successful as a Home Care Practical Nurse at Amedisys, you will need:

• Current LPN license, specific to the state(s) you are assigned to work
• One year of clinical LPN experience
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance
