About the position
Responsibilities
- Execute nursing interventions by collaborating with the care team and following established standards of care.
- Assess and collect data related to patient health status and develop care plans with the care team.
- Utilize data to meet care coordination needs across settings in conjunction with patients and families.
- Manage nursing care for individual patients, ensuring accountability and responsibility.
- Document nursing assessments and interventions comprehensively.
- Identify patient and family learning needs and educate them using the teach-back method.
- Provide safe, ethical, and evidence-based patient care that is culturally congruent.
- Perform telephone, My Chart, and in-person triage for symptom management.
- Review and act on test results and manage communications requiring RN education and training.
- Provide patient education through various channels including in-person, telephone, or video.
- Assist with medication management and complete timely documentation in the electronic medical record.
- Manage urgent and emergent patient care situations and participate in care coordination activities.
- Establish standard clinical supply storage and manage medication/immunization inventory.
- Float to other clinics as needed and serve on relevant committees and task forces.
Requirements
- Bachelor's degree in Nursing or equivalent education, training, and experience.
- Four years of related experience.
- Current and unrestricted New York State RN license.
- BLS certification.
- Ability to work in an environmentally-controlled indoor setting.
- Ability to perform sedentary work involving long periods of sitting or standing.
- Ability to travel between clinical sites as needed.
- Exposure to blood, bodily fluids, and tissue, with the ability to perform emergency care as needed.
- Excellent communication skills and ability to work collaboratively with a diverse population.
- Strong proficiency in computer skills including Electronic Health Systems and Microsoft Office.

Benefits
- Competitive salary range of \$98,000 - \$107,000 based on qualifications and experience.
- Regular work schedule of 40 hours per week, Monday to Friday, 8 AM to 5 PM.
