About the position
Responsibilities
• Conduct in-home, in-facility, or telehealth assessments using the UAS-NY assessment tool.
• Establish and maintain positive relationships with consumers, caregivers, and facility staff.
• Regularly review medical documentation and report any concerns or conflicting information.
• Enter assessment data into the UAS-NY online application and transmit as required.
• Provide direct supervision to assigned nursing and clinical staff.
• Perform clinical case reviews and consultations.
• Provide education and clinical training to staff in area of expertise.
Requirements
• Current Registered Nurse (RN) license valid in the state of New York.
• Bachelor's degree and 5 years of experience or equivalent combination of education and experience required.
• Minimum of 5 years of clinical experience.
• Minimum 1 year supervisory experience preferred.
• 2+ years RN experience in-home care.
• Ability to travel locally and reliable form of transportation.
Nice-to-haves
