About the Role Our client, a leading national provider of integrated health and social services, is seeking dedicated and experienced Remote Case Managers to join their expanding virtual team. You will be responsible for assessing client needs, developing personalized care plans, and coordinating a comprehensive array of services to promote well-being and ensure access to care. Key Responsibilities Conduct comprehensive virtual assessments of client needs, including medical, social, and financial factors. Develop individualized care plans in collaboration with clients and their families. Coordinate referrals to appropriate community resources, healthcare providers, and social services. Monitor client progress and adherence to care plans, making adjustments as necessary. Serve as a primary point of contact and advocate for clients, ensuring their needs are met. Maintain detailed and accurate client records in accordance with organizational policies and regulatory requirements. Communicate effectively with a multidisciplinary team of healthcare professionals and service providers. Educate clients and families about available resources and healthcare options. Respond promptly to client inquiries and manage caseload efficiently. Participate in virtual team meetings and professional development activities. Requirements Bachelor's degree in Social Work, Public Health, Sociology, Nursing, or a related field. Minimum of 3 years of experience in case management, social work, or health services coordination. Strong understanding of healthcare systems, social services, and community resources. Excellent assessment, planning, and problem-solving skills. Proficiency in utilizing telehealth platforms and electronic health record (EHR) systems. Exceptional verbal and written communication skills. Ability to work independently, manage time effectively, and maintain a high level of organization in a remote setting. Must have a reliable high-speed internet connection and a dedicated, private workspace. State-specific licensing or certifications may be required depending on service area.
