Position Summary
We are seeking a highly skilled and compassionate Health Coach Consultant RN to join our team in North Carolina. As a Health Coach Consultant RN, you will work with our members to assess their health needs, develop personalized plans to achieve optimal health, and provide ongoing support and education to help them make informed decisions about their health. This is a full-time, 40-hour per week position with a flexible schedule that includes rotating weekends and evenings.
Key Responsibilities:
- Assess members' health needs through comprehensive evaluation of clinical data and information
- Develop and implement personalized plans to help members achieve optimal health status
- Provide ongoing coaching, education, and support to members to promote healthy lifestyle choices and behavior changes
- Utilize clinical guidelines and criteria to positively impact members' health outcomes
- Collaborate with supervisors, Medical Directors, and other stakeholders to overcome barriers to meeting goals and objectives
- Identify and escalate quality of care issues through established channels
- Document clinical notes and plans of care in an electronic medical records (EMR/EHR) system
Requirements
Essential Qualifications:
- Unrestricted RN compact license in the state of North Carolina
- 3+ years of recent clinical experience in Adult Medical Surgical floor nursing
- 3+ years of recent clinical experience providing health education about chronic health conditions to adults
- 3+ years of experience with documenting clinical notes in an electronic medical records (EMR/EHR) system
- 3+ years of proficiency with Microsoft Office applications: Word, Outlook, Excel, Teams, and PowerPoint
- 2+ years of experience delivering health education coaching via telephone 1:1
- Associate's or Bachelor's degree in Nursing
Preferred Qualifications:
- Previous telephonic chronic condition coaching experience
- Virtual group coaching experience via webcam
- Managed care experience
- Experience with Epic EMR database functionality and documentation
- Excellent communication skills and comfort with public speaking
- Ability to work independently as well as part of a group
