This position functions remotely, but requires strong clinical judgment, communication skills, and a collaborative mindset to ensure continuity of care.</p> <h3><strong><br></strong></h3><h3><strong>Your Day-to-Day Work</strong></h3> <ul><li>Provide nurse telephone triage using clinical judgment, standardized protocols, and decision-making tools.</li><li>Assess patient symptoms and determine urgency of intervention, escalating to providers as needed.</li><li>Educate patients and families about diagnoses, treatments, and prescribed medications.</li><li>Review and triage abnormal cardiac test results, follow through on patient problems like Afib, blood pressure, and post-operative concerns.</li><li>Document interactions accurately in the EMR and maintain compliance with HIPAA and CSC standards.</li><li>Serve as a clinical liaison between providers, community partners, and clinic staff.</li><li>Function as a resource for patients, families, and support staff in cardiac health.</li><li>Collaborate in policy and workflow development to improve care delivery.</li></ul> <h3><strong><br></strong></h3><h3><strong>Environment and Shift Details</strong></h3> <p>This is a <strong>Per-Diem remote</strong> position, with initial <strong>onsite training required in Olympia WA</strong>. Occasional travel and intermittent onsite attendance may be required.
